
​​​​​​​​How to Make Your Doctors Note or Medical Request easier and more efficient

Charlestown Family Health Practice offers a wide range of custom medical and legal reports for patients. We understand that sometimes you may need a report for work, insurance, legal matters, or other important reasons. We are happy to help — but there are some things you can do to make the process smoother and quicker.
To help us complete your report without delay, please follow these tips:
Tell us who needs the report
Is it your employer, solicitor, insurance company, school, or someone else?

Explain why the report is needed
Is it for a court case, accident claim, travel cancellation, or something else?

Include any letters or forms you were given
If the person or company asking for the report gave you a letter, email, or form — please include this. It helps us understand the context, what’s needed and avoids delays.

Allow enough time

Simple doctor notes (1 week)

We aim to complete all doctor notes within teh working week. We aim to complete them as quickly as possible, but urgent requests are not always possible. Please plan ahead if you have a deadline.

Medical reports (2 - 4 weeks)

Ask if there is a fee
Many reports are not covered by the HSE or your medical card. We will let you know the cost when you ask at reception.
Reports We May Not Be Able to Provide
There are some reports that GPs cannot usually provide, or where we may not be the right person to comment. In these cases, we may suggest contacting another service (like an occupational health doctor or social worker). Please check with reception if you’re not sure.
