Help Us Keep Your Record Up to Date

We are updating our systems 

 

What Is This About?

Between October 2025 and January 2026, we are carrying out an important project to update patient records.  This is about keeping your information accurate and safe, so we can:

  • Contact you easily

  • Send referrals to the correct place

  • Keep your next of kin (emergency contact) up to date

  • Make sure you’re offered services like flu vaccines, online  access for ordering scripts, and more

 

What Will Happen?

Over the next few months, we will try to contact every adult patient in the practice.

We may contact you:

  • By phone

  • By text message

  • Or when you visit the practice

You can also choose to:

  • Fill in a form online  

  • Ring us on the practice number

  • Or pop into reception to fill out a paper version

 

Why It Matters

Keeping your information up to date:

  • Prevents delays in test results and referrals

  • Helps us contact the right person in an emergency

  • Makes sure you only get contacted in the way you prefer

  • Ensures we are meeting legal and privacy rules (GDPR)

 

What Will We Ask You?

We will ask you for a few simple details:

  1. Your name and date of birth

  2. Your address and phone number

  3. Your next of kin (someone we can call in an emergency)

  4. Your email address

  5. Consent preferences  

  6. Are you interested in our online app — PIPPO?

    PIPPO lets you request repeat prescriptions and book appointments online securely.

  7. Any feedback or comments for us?

    1. Tell us what you like, what we could do better, or anything else you think we should know.

 

Timeline

  • Starts: October 17, 2025

  • Ends: January 31, 2026

  • We will work alphabetically over the four months.

 

What About Privacy?

We are committed to protecting your personal data.

  • Your information will be handled under GDPR (General Data Protection Regulation)

  • Stored securely on our clinical system (Helix / Socrates)

  • Only shared with people involved in your care

  • Texts and emails will only be sent if you give separate written consent (see below)
