Your Personal Health Check

​

At Charlestown Family Health Practice, we want to support you live a fulfilling, healthier life.

​

That’s why in December 2024, we started the HSE CDM programme, which in other words is your 'Personalised MOT Programme'.  

 

This personalised MOT is designed to:

  • Catch early signs of heart and lung disease

  • Monitor and manage long-term health conditions

  • Support you in setting health goals and staying on track

 

We offer this service to both public (medical card) and private patients. The type of check-up you are offered depends on your age, your medical history, and whether you have a medical card or are attending privately.

 

For Public (medical card) Patients​

 

Free Screening at Age 45+

(as part of the HSE chronic disease management programme)

 

If you are a medical card patient over 45 and have no history of long-term illness, you are entitled to a free health check every 5 years.
At this appointment, we will check for:

​

  • High blood pressure

  • High cholesterol

  • Diabetes (high blood sugar)

  • Liver disease, thyroid disease, renal disease

If your results are normal, we will invite you back in five years.

​

 Annual Prevention Programme

(as part of the HSE chronic disease management programme)​

 

If you are a medical card patient over 18 and have risk factors for diabetes, heart disease or stroke, we will invite you to join our Prevention Programme. This includes people who have:

​

  • High blood pressure

  • A history of gestational diabetes (diabetes during pregnancy)

  • A family history of heart problems or stroke

  • Other risk factors (e.g. smoking, overweight, inactive lifestyle)

 

What’s included:

​

  • Annual blood tests

  • Vital sign checks (blood pressure, weight, heart rate)

  • A review with a clinician to:

    • Set health goals

    • Review or adjust medications

    • Offer lifestyle advice​

 Twice-Yearly Reviews for Chronic Conditions (as part of the HSE chronic disease management programme)

If you have a known heart or lung condition, you will be invited for two check-ups per year.

Conditions that qualify include:

  • Atrial fibrillation (irregular heartbeat)

  • Heart failure

  • Previous heart attack

  • Narrowing of the heart arteries (ischaemic heart disease)

  • Previous stroke or mini-stroke (TIA)

  • COPD (chronic bronchitis or emphysema)

  • Asthma

  • Diabetes

Your reviews will include:

  • Vital signs every 6 months

  • ECG

  • Annual blood tests (twice yearly with Diabetes)

  • Medication review and health planning

 

 For Private Patients

​

We are pleased to offer the same high-quality programmes to private patients.

​

We try to align your check-up with the month before your birthday, but we are flexible to suit your schedule.

This includes:

  • Screening and early detection

  • Annual or twice-yearly reviews (depending on your condition)

  • Blood tests and clinical checks

  • Lifestyle advice and medication review

  • Shared health goals for long-term wellbeing
